AIB presents Tema aircraft crash investigation findings

The Aircraft Accident and Incident Investigation and Prevention Bureau, AIB Ghana, has revealed major operational and maintenance lapses linked to the fatal Tema aircraft crash that claimed two lives on March 16, 2026.

According to Head of Investigations, Paul Fordjour, all three flights conducted from Ho Airport on the day of the crash were unauthorised and operated without valid permits.

Investigators say neither Air Traffic Control nor airport authorities in Ho and Accra had full knowledge of the flight permissions issued for the aircraft, exposing serious gaps in aviation oversight.

Although the pilot was properly licensed, the Bureau noted that flight operations significantly deviated from approved procedures.

The report also revealed that the aircraft had no designated maintenance facility or assigned personnel responsible for its upkeep, while previous signs of engine overheating had not been properly addressed.

According to investigators, the aircraft most likely suffered engine failure caused by persistent overheating, which played a major role in the crash.

AIB Ghana further disclosed that no distress call or emergency declaration was made before the aircraft went down.

The Bureau has since issued recommendations to strengthen aviation safety, improve regulatory compliance and enhance emergency reporting procedures.
